To reorder dental or vision cards on an RMI-Sponsored plan with Ameritas, you can either contact Claims Customer Service at 800-487-5553, or you can view or print a copy of your card(s) from Ameritas’ website. To access your card(s) through the website:
- Go to ameritas.com and then click on “Sign In” in the upper right corner.
- In the middle part of the page, the “Personal Accounts” tab should be selected if not already open. Then in the “Dental/Vision/Hearing” section, click on “Member Sign In.”
- A new screen will appear that shows an option for existing users and for first-time users.
- If you are creating an account for the first time, click on “Register Now.” Then follow the prompts to register and set up your preferences.
- If you have previously registered and already have an account, enter your user ID and Password, then click on “Sign In.”
- Upon signing in as existing user, click on the “Dental” or “Vision” link at the top of your homepage to see the options for that benefit.
- Click on the “ID Card” Icon.
- Click on the “View, print or save” link to access your ID card.
