To reorder dental or vision cards on an RMI-Sponsored plan with Ameritas, you can either contact Claims Customer Service at 800-487-5553, or you can view or print a copy of your card(s) from Ameritas’ website.  To access your card(s) through the website:

  1. Go to ameritas.com and then click on “Sign In” in the upper right corner.
  1. In the middle part of the page, the “Personal Accounts” tab should be selected if not already open. Then in the “Dental/Vision/Hearing” section, click on “Member Sign In.”
  1. A new screen will appear that shows an option for existing users and for first-time users.
    1. If you are creating an account for the first time, click on “Register Now.” Then follow the prompts to register and set up your preferences.
    2. If you have previously registered and already have an account, enter your user ID and Password, then click on “Sign In.”
  1. Upon signing in as existing user, click on the “Dental” or “Vision” link at the top of your homepage to see the options for that benefit.
  1. Click on the “ID Card” Icon.
  1. Click on the “View, print or save” link to access your ID card.
